This is a Performa (Disk Tools) Utilities disk at version 7.1P4. It's
a bit of an oddity, because Apple officially skipped from 7.1P3 to
7.1P5 for official Performa installations, but the restore disk came
in at 7.1P4.

Mounted Name: Utilities
Date: 1993
Version: 7.1P4
Part Number: 690-0989-A

Labelled "Utilities" it is = to the "Disk Tools" floppy of standard
Macintosh install disks of the same period.

Compatibility
Architecture: 68k

Performa 450 through 600 computers, Basilisk II with Quadra/Performa
ROM.

Best used with actual hardware. Macintosh emulators do not emulate
hard drives to a degree acceptable by the "Apple Restore" program (see
pic #4 above).

Screenshot #5 is of a full installed version desktop and does not
represent the current contents of what's here, at present.
