 How the state handles it: Delaware's program, in place since 2011, reports detailed reviews of mothers' deaths and doesn't shy away from medical failures. The team’s findings are published in the same reports that examine children’s deaths and near deaths, and children are the main focus of those reports. The 2017 report, for example, investigated the causes of five maternal deaths during pregnancy or up to one year postpartum; only one was linked to pregnancy or its complications, while the other four were either homicide or acute drug intoxication.
