Social worker with a history of sanctions seeks license reinstatement [1]

The Iowa Board of Social Work held a hearing this week to consider the possible reinstatement of a license for a woman previously sanctioned for having sex with a client.

The notice of the hearing was issued by the board last month, but was only recently made public.

State records indicate that in March 2012, while social worker Lisa Bailey was employed by the Mental Health Center of North Iowa, she was convicted of drunken driving, leaving the scene of an accident, and causing personal property damage. Bailey allegedly told a supervisor she had been “black out drunk” at the time, but testified in court that she had not been drunk.

Later, the trial judge reportedly informed Bailey’s supervisor that a witness was alleged to have seen Bailey crash her car into another vehicle, walk away and then “stumble” her way into a convenience store where she purchased beer. Bailey was subsequently fired from the center, according to board records.

In April 2013, Bailey was hired as a full-time outpatient counselor for Prairie Ridge Addiction Services. That same year, the board denied her application to practice master’s level social work independently, citing concerns with her ability to maintain professional boundaries with clients and colleagues. A supervisor at the Mental Health Center had alleged Bailey had been seeing a client on a regular basis during the evening hours when the center was closed.

In 2014, the board charged Bailey with engaging in a dual relationship with a current client, engaging in a sexual relationship with a current client, practicing while experiencing a mental or physical impairment, and engaging in unethical conduct. The board alleged she had made numerous phone calls and sent numerous text messages to a client and engaged in a sexual relationship with that same individual. Bailey later agreed to surrender her license.

The board has yet to make public a decision on Bailey’s application for reinstatement.

Another state licensing board, the Iowa Board of Nursing, recently filed charges against several licensees, including:

Sadessa Hernandez of Altoona, who is charged by the board with falsifying records related to nursing practices or knowingly permitting the use of such records, and with failing to properly document or perform the disposal of medication.

Trenton Buchanan of Cedar Rapids, who is charged by the board with committing an act that might adversely affect a patient’s welfare; misappropriating medications, property, supplies, or equipment for a patient; being involved in the unauthorized possession or use of a controlled substance; and excessive use of drugs that might impair a licensee’s ability to practice the profession with reasonable skill and safety.

Sarah Kreft of Polk City, who is charged by the board with failing to comply with an unspecified order previously issued from the board. The allegations are tied to a law that prohibits a nurse from knowingly making misleading or untrue representations in the practice of a profession or engaging in unethical conduct.

Cherish Kueker of Waverly, who is charged by the board with failing to respond to the board during a board audit or failing to submit verification of compliance with continuing education requirements.

Bonnie Herrin of Newton, who is charged by the board with failing to respond to the board during a board audit or failing to submit verification of compliance with continuing education requirements.
