NTSB Preliminary Report on East Palestine Train Crash [1]

The NTSB (National Transportation Safety Board) has released their preliminary report on the East Palestine, Ohio Norfolk Southern train derailment. on February 3, 2023. NTSB has determined the accident was caused by a faulty axle. The train (158 cars, not mentioned in report) was a "mixed freight train" with an uneven weight distribution.

Originally departing Morton, Illinois the train broke down twice between South Bend & Mishawaka, Indiana. It was "patch routed" to Toldeo, Ohio & received repairs . While in Toledo, the crew was changed & the train was cleared to continue to Conway, Pennsylvania. However, the NTSB was unable to obtain maintenance repair orders for the train.

At 7:38 PM EST, the first 911 call reporting the train on fire was made in Sebring, Ohio, approximately 30 miles before derailment, to the Mahoning County 911 system & was immediately transferred to the Columbiana County Emergency Management System. Four more 911 calls were made about the train being on fire. Two videos show flames from the train as it passed through Salem, Leetonia, Columbiana & New Waterford, Ohio before derailing in East Palestine. Approximately six miles before the derailment, a wayside sensor was tripped & the crew began braking procedures.



It is still unknown how much & what chemicals were spilled by the train crash. 50 train cars were involved in the crash.

At 10:58 PM EST, Norfolk Southern reported the accident to the Natonal Response Hazardous Material Center. At 11:20 PM EST, the first evacuation notice was issued for a one mile radius from the crash site. The following day, Saturday, February 4, 2023, a mandatory evacuation of a one mile radius was issued due to fears of an explosion. The rail car that was in danger of exploding was "decompressed" resulting in a large cloud of unknown toxins being released.

Three days after the crash, the railroad tracks were repaired & trains began operating again.
