Comments:

MAP03 - (Assigned, Easy) Simple, did this on my first attempt. I start
to do the 'dance' in the room with the backpack, but note I start
backwards. So I shake my head around, exit the room, and start it from
  scratch.

MAP05 - (Assigned, Medium) Also simple, did this on the first try as
well. Walked out hurting, but decided it wasn't worth it to wander for
the abundance of stimpacks remaining. Did the dance right near the end,
  after killing one cacodemon.

MAP28 - (Assigned, Hard) Decided I didn't like doing this, after getting
worked over by the pain elementals and the like. So I opted to substitute
this with 3 other levels from the "Hard" set (MAP06, MAP07, MAP15).

MAP06 - (Substitution #1, Hard) Not hard really, I finished this first
time. Wasn't careful, so I grabbed the megasphere early, rather than at
the end, so I didn't walk out with 200/200. I dance while waiting for
the crusher to kill the big, nasty spider.

MAP07 - (Substitution #2, Hard) Not difficult either, but one of my
favorite levels nonetheless (see my D2SG free-interp!). Plenty of ammo
to go around, and I walk out with the 200/200 health an armor. Dance is
  at the beginning.

MAP15 - (Substitution #3, Hard) Industrial Zone is also another really
fun level, so I opted to do it GM-style. Definitely a level where planning
the route ahead of time helps out. Ended up with the nice 200/200, using
the exit to the secret level. Dance is at the beginning.

MAP12 - (Base expander, rated hard) I chose The Factory as my expander
because it's a level I hadn't played much before. Definitely an exercise
in ammo-conservation and getting monsters to battle each other. After
completing the level, a friend reminded me that there's a switch I missed
that would have made killing the imps near the soulsphere easier - I
jumped into close-quarters with them, and lit off 2 BFG shots instead. The
last room was nerve-racking, so I kinda did the duck and cover thing for
awhile. After all were dead, I spend a few minutes thrashing around
looking for the last switch to let me out. Total time was around half
an hour, so this might not be too exciting to watch. I was torn between
calling this medium and hard, but it was definitely harder than 3 of
the 4 'hard' GM levels I played, so 'hard' it is. Dance is at the
  beginning.

MAP28 - (Free interpretation) Since I opted out of this level for my
assignment, I decided that doing it on Nightmare would be fun. Mostly
as exercise in planning when to get Invulnerability, a fun level
nonetheless. I get out in 2:16 (par 7:00!), with a little "assistance"
from an Arch-Vile. Fun to watch this one in overhead view with monsters
and shots visible (iddt, iddt twice in map-mode) - if I only know how
much was going on during the actual recording! Dance is at the beginning.
