Software Bugs I Have Known


I'm looking for a dramatic event which has resulted from a
software bug. The event could be real or fiction (preferably
real) and could be serious or humorous (the latter preferred).

It will be used as a scene setter for an article on debugging
so will need a reference to an article or a book.

A local person suggested "Apple Crunch", a science fiction
book about a giant traffic jam in New York City which happened
when all the traffic signals went green simultaneously due
to a software bug. However, I have been unable to find this book.
