RECORD REQUEST REQUIRED SEARCH CRITERIA
Nationwide Searches --
1. Address Update Individual Name or Full Address (last available),
including Zip Code Provides most current address, Social Security number
and year of birth based on credit profiles. $21.50
2. Social Security Number: Track One Social Security Number
A search of national consumer credit indexes to reveal user(s) of a
specified Social Security number and the address that corresponds to each
user. The report date is included. Spouse name and/or employment
information may appear. $22.00
3. Social Security Number: Track Two Social Security Number
Search will identify the user(s) of a Social Security number while also
providing current address information, year of birth and the date such
information was first reported. A spouse's initial is included when
available. $22.00
4. Social Security Number: Track Three Social Security Number
This search matches the user(s) of a Social Security number while
providing current address and former address information from consumer
credit profiles. Most responses will also include employment information
and subject age. $22.00
5. Social Security Number: Track All Social Security Number
A search of the three major credit bureaus to match the user(s) of a
Social Security number. Address information, possible employment and age
information may also be provided. $50.00
6. Business Credit I - Market Identifier Business Name and Full Address
Provides company-reported sales and growth figures along with a company
profile which may identify principals, years in business, and primary and
secondary SIC classification. $50.00
7. Business Credit II - Payment History Business Name and Full Address
Details important financial information about a company,including account
types, established payment terms, payment history and average balances.
In addition, public record information such as UCC filings, judgments,
tax liens and bankruptcies are reported. $70.00
8. FAA Aircraft, Airmen Director N-Number or Individual Name or Business
Name By Owner, By N-Number. Displays a list of aircraft owned by an
individual or business nationwide. $30.00
9. Consumer Credit Report Individual Name and Full Address, Social Security
Number(optional)Provides address and employment information along with a
listing of credit accounts,balances,credit inquiries & payment history.
Subject to Federal Fair Credit Report Act and applicable state law
restrictions. Access to consumer credit reports may not be available
to all persons. Reports will be provided only for these purposes:
(1) For use in connection with a credit transaction involving the
consumer.
(2) For use in employment decisions involving the consumer.
(3) For use in connection with underwriting of insurance involving the
consumer.
You must mail necessary & appropriate hard copy documentation supporting
your request for consumer credit information. Documentation must be
received before The Information Center will issue the desired consumer
credit report. $50.00
10. Consumer Credit Employment Individual Name and Full Address, Social
Security Number (optional) A complete consumer credit report used for
employment purposes as allowed under the Fair Credit Reporting Act. This
report allows the employee/applicant to receive a free copy of his/her
credit report. $50.00
11. National Moving Index Individual Name
Updates an address based on information contained in consumer credit
profiles. Often reveals Society Security number. $21.50
12. Infoscan Individual Name and Full Address, Telephone Number
(optional) Verifies occupant and telephone number for a desired address
while uncovering occupant estimated income and length of residency. Also
provides similar information about neighbors & individuals with the same
last name. $36.00
13. National Postal Address Change Individual Name and Full Address (last
available) Uncovers new addresses based on a three year history of postal
address changes, coupled with publisher information and mailing records
from independent sources. $30.00
14. National Publisher Address Change Individual Name and Full Address (last
available) Provides new address information for individuals based on
changes of address filed with various magazine subscription and publisher
mailing lists. $20.00
15. Neighborhood Search Individual Name and Full Address
Matches occupant, phone number and length of residence at a given address
while receiving similar information about up to 10 neighbors. $20.00
16. Surname Scan Last Name and State;First Initial,Zip Code(optional);
First Name, City (Optional) Matches names, addresses and listed phone
numbers of individuals who share a common surname within a specified
geographic region. $23.00
17. Telephone Reverse Directory Area Code and Telephone Number or Full
Address, By telephone number, by address. Searches either a listed
telephone number or residential address to confirm occupancy & match the
telephone number with the address. $20.00
18. Driver's Record Request Individual Name, Date of Birth and Driver's
License Number. Provides a history of an individual's driving performance
and a description of departmental action taken as a result of tickets or
accidents. A driver's license number match and physical description is
also provided. $22.00
19. Vehicle Registration Request License Plate Number and State
Matches registered and legal owners of a vehicle by the license plate
number or vehicle identification number (VIN). $22.00
20. Civil Docket Scan Individual Name and Date of Birth, State and
County Desired Searches any federal,felony or misdemeanor criminal file
to provide file date, case number, level of charges and disposition.

We reserve the right to hold all check-paid orders 2 weeks before shipping.
Most Cashier's checks and money order requests are shipped within 3-5 days of
receipt of order. All orders without proper shipping and handling costs
