Other Secrets:

Inner Sanctum:
 If you finish Story Mode (beat Mong Zhu), you should get this stage.
Go to the 'Chambers/Secrets' option in Options, then select the "?" and
there will be another menu. There will be five things, anything you
haven't gotten yet appears as a "?". The Inner Sanctum will be at the
top.
 Hold R1 and R2 when selecting a stage in Versus Mode ('Manual' Stage
Select must be on in 'Options' of Versus Mode) and push X to accept
your choice.

Practice Arena:
 If you get all characters and complete Practice mode with each and
everyone of them, you will have access to the Practice Arena in Versus
mode. To see how to access this stage, it's the same thing as above.
   Hold L1 and L2 when selecting a stage in Versus Mode ('Manual' Stage
Select must be on in 'Options' of Versus Mode) and push X to accept
your choice.

Mong Zhu:
 I don't really know how to get him, but I did the first time I played
by beating Story Mode. I believe if you get a rank for Time Attack, you
get Mong Zhu.

The Secret Character Outfits:

 You should know the secret character outfits are just new characters
with the same moves as the character the outfit is for. Hold [Select]
when choosing your character, then push X to accept the choice.

 You can also choose The Bodyguard in Practice Mode. Highlight any
character, push and hold all four of the shoulder buttons (L1, L2, R1,
and R2) and push X to accept the choice. The cursor will automatically
go to Ghostface Killah and the guy will be The Bodyguard.

 The other records don't matter. I played through all of the stages,
never continued, never played the same stage more than once, and got all
of the chambers. I wonder what I'd get if I skipped all of the bonus
stages...for Time Attack.

2. What's the Parental Lockout Code?
 The Parental Lockout Code is...

 If you think I copied PRevolution's FAQ, I didn't. I bought this game
the day after it came out. Be forewarned, if you already have secrets
for the game without the Parental Lockout Code, then you can't have the
Memory Card with your file in Slot 1, otherwise it would load that data
and you'd never get a chance to put the code in. If you put the code
in, then you'll be losing the stuff you earned. It's on you.

 These paragraphs below are of my previous response.

 <clears throat> Sorry, I'm not going to tell you. If you don't have
the instruction manual, then it could mean you got the game illegally,
even if you didn't, so I'm not going to tell you. I want to respect what
Paradox Entertainment did, and that's not distributing the code to
whoever doesn't have the manual. It wouldn't be fair to people who had
to get the instruction manual. If you got a burn of this game, well,
that's just how it is for you, isn't it?
 Actually, if you want the code, check out the GameFAQs Message Board.

3. What are some of your "best" juggles?
 Like I said before, I'm not going to tell my secrets to the whole
world. I did that once with Ehrgeiz, and I'm not going to do it again.
