I use 1/8" aluminum shims that I stack between the carrier and
rear most part of the swinger to keep the 91-92 axle straight. I
haven't thrown a chain since.

Looks like the front left bolt holding the carrier in place is
loose?If so the twisting could easily break the rear plate and
cause slack/misalignment in the chain.

My carrier used to work loose with washers... until I replaced
them with the OEM bolts.

I'd recommend a skid plate.

Something torqued the carrier forward on the left side. You can
see where the stud broke it started to take the thread with it.
I wonder it you picked up a rock in between the chain and
sprocket.....?
